Reading The Avengers, I came across this Comics Code Authority approved sequence: I find it odd, that a suicide bomber would get past the screening, maybe it wasn't explicitly forbidden by the rules (not even the 1954 ones), but it sure looks like an oversight. As far as I know, we don't have any thread dedicated to the code, so how about sharing those slip-ups?

IMO... Everything changed after ASM 96-98 and the CCA became nothing more than a Figurehead.

I don't know if it;s a slip up but I was very surprised at the depiction of casual sex between Rick Jones and a Kree scientist. Captain Marvel #49 Scott Edelman/Al Milgrom/Terry Austin

Just read a panel from Captain Marvel #10 (circa February 1969). There is a scene where he snaps the neck of a pterodactyl to kill it. Seemed a bit much and a tad violent for the times, even if it was a dinosaur.

We also have Spider-Man killing Gog, in January 1971, and he was a person, although not human.
