Short answer: code name unless it's a Marvel team book.
Long answer: Marvel was far more interested in exploring their heroes as actual people, with lives outside of fighting crime. Even still, Peter Parker and Matt Murdock don't tend to address themselves by name all that often, so it's really the Marvel team books where alter ego names get used far far more often than codenames. Heck, in the X titles, Kitty Pryde took a long time to find a codename that "stuck," and Jean Grey went close to two decades without a codename after her 1980s resurrection.

Jean Grey went close to two decades without a codename after her 1980s resurrection. I did not know this ... I did not read much of X-Men, Jean Grey, and any other books after the 80's. I'm glad that they did this.

I know "Marvel Girl" is kind of lame now, and "Phoenix" wouldn't really apply, but it still seemed odd.

The opening credit for the 90s X-Men TV show called her "Jean Grey" as well.

It's a big gap and I'm surprised they never came up with a more elegant solution for Jean Grey.
I will throw in here the Golden Age hero Just 'n' Right whose real name was Justin Wright.

Yeah, depends on the individual character. If there was any ambiguity, I would lean towards the code name, partly because I think we should avoid talking about them as if they were real people, as comics fans sometimes tend to do. I have my favourites like anyone else, but anything that helps maintain a little distance, to remember that they're constructs, not actual human beings, let alone our special friends, is a good thing, to my mind. So as you can imagine, I especially dislike referring to them by their first names.I actually agree with this quite a bit. When I refer to any of them by a name rather than a code name it's always first and last name a la Reed Richards or Kitty Pride. I find the familiarity of referring to characters by their first name and expecting everyone to understand who you're talking about more than a little creepy. Count me creepy then!

This reminds me about a time in high school when my buddy and fellow comics enthusiast Rob Luettgen (whose collection my parents bought for me as a graduation gift) was catching me up on the latest Marvel news. I guess we were being kinda vague, because a girl at the next table was eavesdropping and finally interrupted us to ask, "I knew Crystal and Johnny were having problems, but who's Pietro? Does he go to Lincoln?" When we explained what we were taking about, she hid her embarrassment by calling us losers.
