Captain Ultra!! Captain Ultra has appeared far more than once, too, in issues of Marvel Comics Presents, New Warriors, The Defenders, Thor, Iron Man and Civil War, amongst other places.

The character that immediately jumped into my head when I read the thread title -- although, strictly speaking, he doesn't really count -- is the Mindworm from Amazing Spider-Man #138... His first appearance in ASM #138 is one of my favourite Spidey comics of that era. There's just something fascinating and, dare I say it, endearing about this character -- his weird appearance, his tragic origin story and his somewhat lame powers. He's a pathetic figure, but one that "speaks" to me somehow. Like I say, he doesn't strictly count because he appeared a second and third time in Peter Parker: The Spectacular Spider-Man #35 from 1979 and in Spectacular Spider-Man (Vol. 2) #22 from 2005. That final appearance, in particular, was a really sad tale, in which the character unfortunately died. RIP Mindworm.

I find it impossible to believe that there's a comic character that either Geoff Johns or Roy Thomas hasn't brought back.

I can name at least one: Roy wanted to use the original Wildfire in All-Star Squadron but couldn't because by then, there was already a rather better known character of that name in Legion of Super-Heroes. He created the female Firebrand as a kind of Wildfire surrogate. Wildfire did eventually make a cameo appearance in the Elseworlds limited series The Golden Age, though I don't think she was named.

Although I think this one may have reappeared in an issue of DC Comics Presents about 15/20 years later repeating exactly everything it said in that issue that it did in Superman 196.
The Gorilla Boss of Gotham City
Kind of glad that these guys haven't reappeared recently - I'd hate to think of what DC would do to them now.

There was a Bronze Age Gorilla Boss (though they didn't call him that) done by no less than Jim Starlin during the Dollar Comic days of Detective. Not half bad, as I recall.

Kind of glad that these guys haven't reappeared recently - I'd hate to think of what DC would do to them now. Gorilla Boss appeared in World's Finest 251, Swamp Thing Annual 3 and Batman Adventures 5 & 6. He also appeared in the Batman: The Brave and the Bold episode "Gorillas in our Midst!". Which is awesome.
