I'm probably wrong, but I can't recall Deadman ever changing his look. Then again, would his "costume" even count, technically, since he's a ghost?

That costume has stayed fundamentally the same. There were some minor tweaks in the New 52 version of the character: the upright collar was rounded and lower, and the character's ears were visible.

Mysterio is a villain who I feel has been drawn differently but the elements of the costume are the same (fishbowl head, cape, colours, eye clasps on the cape, the cape, the boots, etc.).

Checking on that, it appears that Jones does include a collar, but it's drawn very weirdly: He does, however, make another tweak by removing the gloves. In an origin recap, Jones draws Deadman "straight", but with white gloves:

I’m pretty sure the Spidey Super Stories Mysterio’s fishbowl was more like a small led. Not too round, but like a plug.

I always assumed it was a colouring error when Galactus was wearing the shorts - for no other reason than that I thought it looked so stupid - but regardless, I hardly even think of it as a costume change, since it's pencilled and inked in exactly the same way.
